Application to buy an FOTTP Memory Leaf

I wish to purchase a stainless steel message leaf for £25 plus engraving costs* to hang on the metal tree in the Friends Sensory Garden in theChelsea Gardens in Telford Town Park

On receipt of your application we will email an invoice for the purchase price of £25 which can be paid either on-line through PayPal or by cheque through the post. And on receipt of your payment we will let you know the arrangements for collecting your leaf from the Town park Visitor Centre and for returning it for hanging after you have had it engraved.

NOTES

* the special offer of engraving with the message of your choice for a flat fee of £10 is available only from the Telford Town centre branch of Timpsons
**You may opt to donate the profits from this sale (minimum £17) to The Friends of Telford Town Park to help with the work of maintaining the gardens, to one of these suggested charities – The Midlands Air Ambulance Charity,Cancer UK, The British Heart Foundation, Alzheimer’s Society,Severn Hospice, Parkinson’s Society, Guide Dogs UK – or to a registered charity of your choice
